Applying for Home Care with Brightwater

You’ve decided on Brightwater for your home care services, thank you! We can’t wait to have you on board.

Here’s how you can apply for Brightwater at Home.

Get Assessed:

To begin the home care process, you are going to need an ACAT assessment. You can organise an assessment through My Aged Care.

Get Approval:

If successful in your assessment for a Home Care Package, you will receive a letter of approval. This letter will include the services you are eligible to receive, a copy of your support plan and anticipated timing for the home care package being allocated.

Get Assigned:

When you have been assigned a Home Care Package, you will receive a letter with the words ‘you’ve been assigned a home care package’.

This letter includes your referral code which is your unique reference number for receiving services which you will have to give to your chosen provider.

You will have 56 days from the date of your letter to enter into a home care agreement with Brightwater.

Give us a ring as soon as you receive this letter to begin the application process.

Get Brightwater:

Our Welcome Team will take all the relevant details for your package assignment during the first call.

We will organise a face-to-face meeting with our coordinators to come and visit your home. They will help you with the paperwork process including the Services Agreement, payment options etc.

Get Your Coordinator:

Once all the paperwork has been completed and agreements signed, your new coordinator will ring you to organise a visit. This will be within 2 to 3 business days of completing the paperwork.

Have a think about a day and time which suits you and any family support you would like to have during the visit. If you need assistance with any translation services, just let our coordinator know.

Your coordinator will develop a comprehensive care plan with you during this visit. This plan outlines your goals, and how we optimise our services with already existing support.

Get Your Nursing Support:

A few days after our coordinator visits, you will receive a visit from one of our Registered Nurses. They will complete a clinical health assessment, which aims to reconfirm your care plan and also pre-empt any future concerns or treatment plans which can be planned for.

This visit will normally happen within 2 to 3 business days after our coordinator comes to visit. Let your coordinator know if you would like to have family present for your nurse visit and be ready with your current medication.

Get Your Services:

Your new coordinator will let you know an estimated commencement of services during your first visit.

Your coordinator will leave their contact information with you so you can call with any questions or need any assistance during this time.
