Brightwater Community Transition Care Program (TCP) supports older adults following a stay in hospital. Clients can include people that have been transferred directly from hospital to live at home, with family members, or from a transition care residential setting. TCP aims to enable older adults to regain confidence and maximise their independence through a low intensity rehabilitation program. The program is time restricted and focuses on developing ‘real life’ skills that are required to carry out everyday activities such as showering, dressing and preparing simple meals.
As our TCP rehabilitation occurs in a real life setting, therapy may look very different from what you or your relative has experienced in hospital. Skills such as getting out of bed, walking to the shower, getting clothes for the day, dressing and preparing meals are all practised in your home.
Brightwater’s TCP provides the following support services to clients living in their own home;
- Personal Care
- Domestic and Meal Assistance
- Shopping and Social Support
- Aids and equipment
- Therapy
- Exercises – Learning how to do tasks
- Help getting to appointments
Our aim is to assist people to improve their confidence and functional abilities in order to return and remain living at home.
At the conclusion of the program you may not need any further help, however if you do, Brightwater will help organise ongoing support.
The Team
The team at Brightwater Community Transition Care work very closely with clients and their families. Together we set personalised goals and work towards accomplishing them, enabling people to continue living at home.
The team includes a Nurse Practitioner, Registered Nurses, Enrolled Nurses, Physiotherapists, Occupational Therapists, Social Workers, Speech Pathologist, Therapy Assistant as well as Support Workers and a Coordinator that ensures each clients individual program is on track.
