Our aim is to assist people to improve their confidence and functional abilities.
Brightwater's Transition Care Service is for older people after their discharge from hospital to enable them to return home, or to await suitable permanent high care or low care accommodation. The Transition Care Program is a short term low intensity therapy support program to help clients improve function and is provided at Brightwater Kingsley and Brightwater Birralee facilities and in the community.
