Brightwater provides Transition Care Service for older people after their discharge from hospital to enable them to return home, or to await suitable permanent high care or low care accommodation. The Transition Care Program is a short term low intensity therapy support program to help clients improve function and is provided at Brightwater Kingsley and Brightwater Birralee facilities and in the community.
Brightwater Discovery Way, Marangaroo, provides interim accommodation for young people with complex health conditions and disabilities, who are inpatients in a metropolitan hospital and have no suitable accommodation available. The facility provides long-term transitional accommodation, assisting individuals to progress to permanent supported accommodation in the community or return home with community services.
