Home and Community Care

Home and Community Care (HACC) services are designed for adults of any age who need only a small amount of assistance, perhaps once or twice a week, to enable them to remain living independently. This can include help with everyday tasks such as showering, dressing, domestic assistance, shopping, paying bills and social support. Nursing services may also be provided if required.

Do I qualify for HACC?
    

You do not need to be assessed by an Aged Care Assessment Team (ACAT) to qualify. You simply contact Brightwater At Home on (08) 9489 5000.

To be eligible for HACC services you must be:
•   living in the community with circumstances (disability or ageing) which affect your 
    capacity to live independently; or
•   be a carer of an eligible person

Brightwater assesses each individual situation to identify the level of support needed and will work together with the individual to provide the appropriate level of support to enable independence.

What is the cost?

 
There is some government assistance available however in most cases, the individual pays for the service. This charge is minimal and if you are on a pension there is a fee cap. The contribution is based on the level of support required.
