Extended Aged Care at Home Dementia

Extended Aged Care at Home Dementia (EACHD) packages are for older people living with dementia who need frequent and high-level support at home. Our Care Workers will help with everyday tasks like showering, dressing, meals, cleaning, shopping, transport and paying bills.

People living with dementia may experience psychological and behavioural changes.  Our staff are highly qualified and experienced in providing individually customised support both to the person living with dementia and their families.

Extended Aged Care can also include the services of a Nurse and other health professionals such as Physiotherapists and Podiatrists depending on your need. To ensure you receive the support you need, Brightwater provides professional Case Management services - and you will also have access to the Government’s Dementia Behaviour Management Advisory Services.

Do I qualify for EACHD?
          

Extended Aged Care at Home – Dementia Packages are subsidised by the Commonwealth Government, so an ACAT assessment to needed.

ACAT stands for Aged Care Assessment Team. They’re experts in determining what level of care best suits each individual.

To arrange an ACAT assessment, please see to your doctor or GP.

If you have recently been assessed and have received a copy of your Aged Care Client Record (ACCR) and it is dated during the past 12 months, you can contact Brightwater At Home directly on (08) 9489 5000.

What is the cost?
                            

The fee for these packages is based on 17.5% of the basic single pension.  However, services will not be denied due to an inability to pay.
